We are so pleased that you have chosen Wall Street Dental to serve your dental needs.  Please take a moment to read this informational page to help inform you about some of our office policies hopefully answer some of your questions and make your visit as comfortable for you as we can.

Please make sure to complete all of the questions so that we may better serve your needs.  Please bring your dental insurance card and driver’s license to your appointment so that we may make a copy for our records.  If any changes in your information occur, please contact our office so we can make those changes on your records.

Appointments:  We realize all of our patients are very busy.  In order for us to keep our appointments running on time, we ask that you keep your appointment with us on time.  This is especially true for our hygienists.  If they do not have enough time, they cannot do a proper cleaning.  We understand that sometimes cancellations are necessary due to conflict and emergencies however, we ask that you give us a 24 hour notice of cancellation so we can accommodate other waiting patients.  If we do not receive a 24 hour notice, you may incur a charge of $35 for a missed hygiene appointment and a $50 charge for a missed dental procedure appointment.  Your appointment time is a block of our schedule that has been reserved for you!

Dental Treatments:  If you have periodontal disease that will require a more intense treatment.  Our Clinical Staff will discuss this with you.  They will instruct you about proper brushing and dental care.

Some tooth aches may require a root canal.  Please be aware that in some cases, root canals can fail and may require an extraction.  Some teeth may require a root canal and or a crown, even after a simple filling.

Once a crown, veneer, bonding or filling is placed, the color cannot be changed without a remake.  The remake will result in a charge.

Crowns, veneers and fillings can chip or break just like real teeth.  It is imperative that you maintain a healthy, balanced occlusion (bite).  Stress, clenching, muscles, teeth and genetics can affect occlusion.  Many people grind their teeth and this can cause damage or break dental restorations.  Wall Street Dental is not responsible for teeth or restorations broken or damaged by these occurrences.

Dental procedures do carry risks.  Some of these risks include:  temporary or permanent numbness, nerve damage, bleeding or/and bruising, allergic reactions, tooth extractions, root canal failure, stiff jaw, sensitive teeth, involvement or the sinus, infection, teeth fracture, sensitive gums, or the tooth may require further treatment.

Emergencies:  Dr. Yeary’s home phone # is (405) 364-8488.  If it is after hours and you have an emergency, please call his home and leave a message.  He will get back with you as soon as possible.
